Why stairs are the ultimate test for your knees

Here is a fact that surprises most of my patients: when you climb stairs, your kneecap absorbs a force equal to 3-4 times your body weight. Going downstairs is even worse — up to 5-6 times your body weight. For a 70 kg person, that is 350 kg of force concentrated on a joint the size of your fist.

That is why stairs are almost always the first place knee problems show up. You can walk on flat ground just fine, but the moment you face a staircase, your knee tells you something is wrong.

At PhysioSthanak in Borivali West, knee pain during stair climbing is one of the most common complaints I hear — across all ages, from 25-year-old gym-goers to 65-year-old grandparents. The good news is that in most cases, the right exercises can fix this without medication, injections, or surgery.

The 4 most common causes of knee pain on stairs

1. Patellofemoral pain syndrome (runner's knee)

This is the most common cause of stair-related knee pain, especially in people under 50. Your kneecap (patella) sits in a groove on your thighbone (femur). When the muscles around it — particularly the inner quadriceps (VMO) and the hip muscles — are weak, the kneecap tracks poorly in this groove, creating friction and pain.

How it feels: Pain at the front of the knee, behind or around the kneecap. Worse going downstairs than upstairs. Aching after sitting for long periods (the "movie sign").

Who gets it: Runners, gym-goers, office workers who sit all day, women (due to wider hip angles), and anyone who has recently increased their activity level.

2. Osteoarthritis

In patients over 50, arthritis is the most common cause. The cartilage cushioning the joint surfaces has worn down over time, and the exposed bone surfaces create pain during weight-bearing activities — especially stairs.

How it feels: Stiffness in the morning or after sitting, grinding or crunching sensation, pain that worsens throughout the day, occasional swelling.

Who gets it: People over 50, those with a family history of arthritis, previous knee injuries, or those who are overweight.

3. Meniscus problems

Your meniscus is a C-shaped cartilage disc that acts as a shock absorber inside the knee. Tears — from injury or wear — cause pain that is often specific to stair climbing because the twisting and loading stresses the torn tissue.

How it feels: Pain on one specific side of the knee (inner or outer), occasional catching or locking sensation, swelling after activity, a feeling that the knee might give way.

Who gets it: Athletes (acute tears from twisting injuries) and people over 40 (degenerative tears from gradual wear).

4. Patellar tendinitis (jumper's knee)

The patellar tendon connects your kneecap to your shinbone. When it is irritated — usually from overuse — it creates a sharp pain just below the kneecap, especially during stair climbing and jumping.

How it feels: Sharp pain just below the kneecap, worse going upstairs or when squatting, tender to press on the tendon.

Who gets it: Runners, basketball players, cricket players, gym-goers who do a lot of jumping or squatting.

Self-assessment — what is your knee telling you?

Before you start exercises, use these tests to get a rough idea of what might be going on:

Sit-to-stand test: Stand up from a chair on one leg. If the painful knee struggles with this, your quadriceps strength is likely part of the problem.

Step-down test: Stand on a step on your affected leg. Slowly lower yourself down. If your knee dips inward (toward the other knee) during this, your hip muscles are weak and your kneecap is not tracking properly.

Swelling check: Compare both knees side by side. If the painful knee looks puffier, especially around or above the kneecap, there is likely some inflammation.

Location check:

  • Pain at the front/around the kneecap → likely patellofemoral
  • Pain on the inner or outer side → likely meniscus or ligament
  • Pain just below the kneecap → likely patellar tendon
  • General stiffness with grinding → likely arthritis
These are clues, not diagnoses. A proper physiotherapy assessment involves specific clinical tests that can pinpoint the exact cause.

5 exercises to strengthen your knees for stairs

These exercises target the muscles that support your knee during stair climbing. They are safe for most knee conditions, but if any exercise causes sharp pain, stop and consult a physiotherapist.

1. Straight leg raises (quadriceps activation)

This is the starting point — it strengthens your quadriceps without bending your knee, which means minimal stress on the joint.

How to do it:

  • Lie on your back, one knee bent (foot flat), the other leg straight
  • Tighten the thigh muscle of the straight leg
  • Lift the straight leg to the height of the bent knee
  • Hold for 3 seconds
  • Lower slowly
  • 3 sets of 15 repetitions each leg

2. Wall sits (isometric quad strengthening)

Wall sits build endurance in the exact range of motion your knees use on stairs.

How to do it:

  • Stand with your back against a wall
  • Slide down until your knees are bent to about 45-60 degrees (not deeper than 90 degrees)
  • Hold the position for 20-30 seconds
  • Stand up and rest for 10 seconds
  • Repeat 5 times
Important: If 45 degrees is painful, stay higher on the wall. The exercise should create a burning sensation in your thigh muscles, not pain in your knee joint.

3. Step-ups (functional stair training)

This exercise directly mimics stair climbing, but in a controlled way.

How to do it:

  • Stand in front of a sturdy step (15-20 cm high to start)
  • Step up with your affected leg, straightening the knee fully at the top
  • Step down slowly with control — do not just drop down
  • 3 sets of 10 repetitions each leg
Progression: As this gets easier, increase the step height gradually. The goal is to build up to a normal stair height (about 18-20 cm) without pain.

4. Clamshells (hip strengthening)

Weak hips are one of the biggest hidden causes of knee pain. When your gluteus medius is weak, your knee collapses inward during loading — on stairs, during squats, even during walking.

How to do it:

  • Lie on your side, knees bent to 45 degrees, feet together
  • Keeping feet touching, lift your top knee open like a clamshell
  • Hold for 2 seconds at the top
  • Lower slowly
  • 3 sets of 15 repetitions each side
Pro tip from the clinic: If you can do 15 reps easily, place a resistance band just above your knees to add difficulty.

5. Terminal knee extensions

This exercise targets the last 30 degrees of knee straightening — the exact range where the VMO (inner quad) is most active and where most stair pain occurs.

How to do it:

  • Loop a resistance band around a table leg at knee height
  • Stand facing the table, with the band behind your knee
  • Start with your knee slightly bent (about 30 degrees)
  • Straighten the knee fully against the band's resistance
  • Hold for 2 seconds
  • Slowly return
  • 3 sets of 15 repetitions

Lifestyle modifications that help

Beyond exercises, these daily adjustments reduce knee stress on stairs:

  • Lead with the strong leg going upstairs, weak leg going downstairs — the saying "up with the good, down with the bad" is a simple rule that reduces load on the painful knee
  • Use the handrail — there is no shame in this. A handrail offloads roughly 20% of your body weight from the knee
  • Maintain a healthy weight — every kilogram of body weight adds 4 kg of force on your knee during stair climbing. Losing even 5 kg makes a significant difference
  • Avoid prolonged sitting — sitting with your knee bent for more than 30 minutes increases patellofemoral pressure. Stand up, straighten your legs, and walk for a minute every half hour
  • Wear supportive footwear — flat shoes and worn-out soles do not absorb impact. Shoes with good cushioning reduce the force transmitted to your knees

When to see a physiotherapist

Most mild knee pain responds well to the exercises above within 4-6 weeks. But see a professional if:

  • Pain is getting progressively worse despite exercises
  • Your knee locks, catches, or gives way — this suggests a mechanical problem (meniscus tear, loose body)
  • Swelling that does not go down within 48 hours after activity
  • Pain at rest or at night — this is unusual for simple mechanical knee pain and needs investigation
  • You cannot straighten or fully bend your knee — loss of range of motion needs assessment
  • Pain after a specific injury — a twist, fall, or direct hit to the knee
